Cardiac Rehabilitation Program

Cardiac Rehab Home Activity Journal

If you are recovering from a heart attack or heart surgery, you’ll need to make some lifestyle changes, though you may not be sure how. Our cardiac rehabilitation program can help.

Cardiac Rehab at Southwest Health gives you the tools to take charge of your heart health—to get well and to prevent another attack.

Our supervised Cardiac Rehab program offers a personalized exercise routine, education about medications and nutrition, and emotional support so you can resume normal activity.

Talk with your physician about Cardiac Rehab. Or, call us for more information at (608) 342-4748.

Cardiac Rehab Phase II

Phase II is an outpatient rehab program designed to reinforce what you learned during hospitalization. Rehab begins one to six weeks after hospital discharge depending on the procedure. Sessions are held three times per week with session length determined by your specific needs.

Cardiac rehab hours are by appointment and include One-hour sessions Mondays, Wednesdays, and Fridays based on your (the patient’s) needs and schedule.
